The Community Health Program is comprised of community-based health care providers who follow-up and offer continued contact with the health care delivery systems at the community level and promote health promotion/disease prevention activities.
We strive to advocate, educate, inform and promote healthy holistic lifestyles, incorporating culturally appropriate values and health concepts within our community. In the provision of services, the Community Health Representatives coordinate and work closely with various Federal, State, County, and Local service agencies (i.e. AHCCCS, ALTCHS, IHS, etc.)
What are the Community Health Representatives (CHRs)?
Community Health Representatives are the frontline of public health workers who are familiar with the strengths and weaknesses of our communities.

All Community Health Representatives strives to provide quality outreach health care and health promotion/disease prevention services to all who resides within the Hopi communities.
Scope of Work
Monitor Clients & Community
Check vitals and help with appointments
Case Management
– Maintain a caseload;
– Provides follow-ups;
– Coordinates referrals;
and consultation with other service providers
Translation & Interpretation
Assist clients to understand treatment orders using native language.
Health Education
Health Screenings
Conduct community screenings to detect potential health risks and coordinate referrals as needed.
Health Promotion & Disease Prevention
Promote and encourage self-care through positive health practices/activities to achieve optimal health; and to encourage annual check-ups.
Do Community Health Representatives Provide Transportation?
Community Health Representatives DO NOT provide transportation for clientele. Please schedule an appointment with the Hopi Medical Transportation Program housed in the same office at (928)737-6351.
